Menarche is the first sign of puberty in adolescent girls and often causes anxiety due to a lack of knowledge and readiness in facing bodily changes. Therefore, menstrual education is needed from an early age. This study aims to determine the effect of health education using an educational monopoly media on anxiety levels in facing menarche among 5th–6th grade female students at SDN 2 Curungrejo, Malang Regency. This study used a pre-experimental design with a one group pre-test post-test approach. The study population consisted of all female students who had not yet experienced menarche, with a total sampling technique of 24 respondents. The instrument used was a modified STAIC questionnaire. The results showed that before the intervention, most respondents experienced moderate anxiety (58.34%), while after the intervention, the majority were in the mild category (58.34%). The average anxiety score decreased from 50.83 to 28.63. The Wilcoxon Signed Rank Test showed a p-value of 0.000 (p < 0.05), indicating a significant difference between pre- and post-intervention. Thus, it can be concluded that the use of educational monopoly media has a significant effect on reducing anxiety in facing menarche. Educational monopoly can be utilized as an alternative learning method in reproductive health education in elementary schools, as well as an engaging, effective, and easily applicable educational tool to improve students’ readiness in facing menarche
